Subject – Medical follow up letter to patient after operation

Dear Mrs. Perkins,

Hope this letter finds you in good health. This letter is a follow up on the operation that was performed on you to remove your gall stones. Your file with the department states that you were supposed to come in every three weeks for a routine check up. But, from your records, it has been seen that you have missed an appointment. Since gall stones are treatable with an operation, it is a common misconception that you do not need a post operation check up and follow up after such a surgery.

It is always advisable to never miss out on appointments so that your doctor can track how your body is responding to the surgery or if the postoperative medication is working or not. Since, we have an interest in the health of our patients; we thought it necessary to draw your attention to this fact. We don’t need to tell you that the more you take care of your health, the better you will feel about yourself.

Hope to see you soon for your scheduled check up.

Yours Sincerely,

Norma Jean
